NHS Support Worker In London: Sponsorship Available

Job Summary

The Transition Support Worker will be a vital member of a multidisciplinary team within the AMHS service, focusing on supporting individuals following discharge from acute mental health care. This role targets patients with frequent admissions, aiming to minimise the risk of future readmissions through tailored, person-centred support.

In this capacity, you will facilitate a smooth transition back into the community by providing cohesive, person-centred care that ensures continuity of support. You will assist patients identified by the Clinical Lead and Transition team, offering both practical and emotional support to help them achieve greater independence. Your approach will be solution-focused, prioritising the specific needs and preferences of the individuals you support.

Main duties of the job

Work with individuals assigned and prioritised by the Clinical Lead, providing tailored support and ensuring effective discharge planning and community integration.

Facilitate smooth transitions from acute care to community settings by assisting with discharge planning, prevention planning, and identifying triggers and coping mechanisms.

 

Link service users with relevant community groups and resources, promoting social inclusion and engagement. Provide recovery-focused support and assist with accessing opportunities both within the Trust and the wider community.

Build and maintain respectful and supportive relationships with service users, incorporating their own goals and support needs into recovery plans. Ensure interactions are sensitive to their ethnic, cultural, and social backgrounds.

Person Specification

Qualifications

Essential

  • Educated to GCSE level or equivalent.
  • Competence in using a range of IT systems for recording and managing patient information.

Desirable

  • Educated to A level or higher level of further education.
  • BSc (Hons) or equivalent in a relevant field, such as healthcare or community services.

Experience

Essential

  • Experience as a support worker, with a solid understanding of patient support and transition processes.
  • Strong communication, interpersonal, and listening skills, with the ability to engage effectively with patients and their families.
  • Excellent organisational and case management skills, with a track record of managing multiple tasks and priorities efficiently.

Desirable

  • Experience of joint working in a multiagency environment, collaborating with various stakeholders to deliver integrated care.
  • Experience in coordinating patient care and discharge planning.

Knowledge/Skills

Essential

  • Understanding of recovery-focused principles and their application in supporting service users.
  • Understanding of the issues and concerns that service users may have in relation to their care
  • Sensitivity to different cultures and socioeconomic conditions, ensuring equitable support for all patients.
  • Effective written and verbal communication skills, with the ability to build rapport with service users, carers, and colleagues
  • Ability to work independently and as part of a team, with skills in facilitating group activities.

Desirable

  • Relevant current legislation, NICE guidelines, CPA, risk assessment and risk management
  • Ability to apply evidence-based practices to improve support outcomes.
