Band 3 NHS Health Care Assistant Job Summary
We have an exciting opportunity to recruit a highly motivated Health Care assistant to join our busy Community Heart Failure service at Liverpool University Hospital NHS Foundation Trust.
This is an exciting post , within a well-established team of specialist nurses. We are looking for an experienced Health Care Assistant with skills in phlebotomy , recording patients clinical observations
This will also require consideration of skills to provide, delivery of patient education, support to reduce symptoms and prevent unnecessary hospital admission/ readmission and maximise the patient’s quality of life.Clinical support will be provided by the Heart failure specialist nurses within the team
The service operates across a variety of settings including community based Heart failure clinics, patient’s home, working closely with primary and secondary care.
Candidate needs to be mobile and have the ability to visits patients in their own home across liverpool in a timely manner.
The role requires flexibility, working within an evolving proactive, dynamic and committed multidisciplinary community Heart failure Team.These is a permanent position

Main duties of the job
The postholder will work as part of the multidisciplinary team and will assist andsupport the clinical team(s). This may include working within the patient’s homeenvironment and other clinical settings within community.
