Therapy Assistant Job Summary

The therapy assistant will engage in Intermediate care.  Intermediate care is a multidisciplinary service that helps people to be as independent as possible. It provides support and rehabilitation to people at risk of hospital admission or who have been in hospital. It aims to ensure people transfer from hospital to the community in a timely way and to prevent unnecessary admissions to hospitals and residential care.

Reading Intermediate Care is a team of physiotherapists, occupational therapists, a nurse, and senior therapy assistants, who treat people in their own homes to rehabilitate them to as high a level of independence as possible. This supportive multidisciplinary team work with patients across Reading in close communication with social services, voluntary agencies and GPs. The team also support the Rapid Response Service, providing support to patients at risk of hospital admission due to lack of equipment or care support.

Therapy Assistant Main Duties

The Senior Therapy Assistant will work with and support the clinicians by working with the patients to achieve their goals. They will also receive and process referrals, collect relevant data information and share information with the relevant services.

The post holder will support and teach practical skills to junior therapeutic and care assistants, students, patients their relatives and/or carers.

Person Specification

Education / Qualifications / Training:

Essential

  • NVQ4/ Or Equivalent qualification
  • Basic computer course
  • ECDL

Desirable

  • Healthcare qualification – Foundation level

Continuous Professional Development:

Essential

  • Evidence of on-going professional development/relevant courses
  • Evidence of statutory & mandatory courses.

Previous Experience:

Essential

  • Minimum of 3 years community /nursing /care experience.
  • Palliative Care Experience
  • Experience of caring for the Elderly
  • Proven working experience of one or more of the following: Intermediate Care Assistant Experience, Health care assistant experience, Occupational therapy assistant experience or Physiotherapy assistant experience
  • Experience of shift lead

Knowledge, Skills & Abilities:

  • Effective written & verbal communication skills.
  • Ability to deal with and communicate sensitive or difficult issues over the telephone or face to face.
  • Ability to work alone and as an effective part of a team
  • Ability to perform technical skills.
  • IT skills and experience

  • Experience of promoting public health.

  • Demonstrate ability to receive and process referral information and share information with relevant services

Additional Requirements:

Essential

  • Car Driver. Full UK Driving Licence (manual)
  • Have use of Car to travel to a variety of settings
  • Reliability and an ability to work consistently to the standards expected
