Health Care Assistant Job summary
We have an exciting Health Care Assistant opportunity to join our Community Nursing Service as a Band 3 Community Health Care Assistant currently based at Orbital in Swindon. You will work seamlessly in collaboration with a variety of community services with the opportunity to provide care for a wide variety of patients.
We have a strong focus on flexible and agile working balanced alongside provision of high-quality services. We provide a 7-day service 07:00- 22:00 with an expectation that your working pattern will support this. This role offers the opportunity to flex across our community teams including Swindon End of Life Care Team (SELeCT) urgent community response, community nursing and Hospital at Home. Ensuring our patients receive the right care, by the right person, in the right place, at the right time is at the heart of what we do. Utilising this approach, we believe will enable you to maximise your clinical skills, development opportunities and job satisfaction.

Main duties of the job
As a Community Health Care Assistant, you will;
- Deliver care/treatment within a community setting, delegated by a registered practitioner and following an appropriate care plan.
- Keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.
- Be proficient in the use of electronic patient record system, have excellent communication and documentation skills that are clear, concise and accurate.
- To work independently within the community setting, visiting patients within their own home.
