The Community Nursing Teams in Brent are seeking HCA responsible for providing clinical in the delivery of high-quality care for the patients in the community setting.

There will be an opportunity to do the Apprentice Nursing Associate training which will lead you to become a Band 4.

 

Main duties of the job

You should hold NVQ level 3 in health and social care and have a minimum of 12months acute or community experience. If you are a motivated team player and wantto excel in delivering exceptional holistic healthcare to a diverse patient group intheir own home, we want to hear from you.

Previous experience of working in the NHS is essential, community experience isdesirable but training will be given. Car driving is not required as we walk to visit ourpatients.

As a Health Care assistant, you will be directly involved in facilitating and shapingself-care for patients with long term conditions in order to help keep them as well aspossible in their own homes.

Qualifications

Essential

  • Higher Professional Diploma in Health and Wellbeing (NVQ/QCF-Level 3) or a recognised equivalent course
  • Willingness to undertake further training as appropriate

Desirable

  • GCSE grade 9-4 or equivalent in English and Maths
  • Registration with the national regulatory body (when it becomes available).

Experience

Essential

  • Experience of working in a health or social care setting
  • Experience of working as part of a team
  • Experience with the use of Microsoft Office and a variety of databases.

Desirable

  • Experience of working as part of a team
  • Experience with the NHS patient information database (Emis, CIS)

Skills & Knowledge

Essential

  • Knowledge of NHS Demonstrate an acceptable level of numeracy and literacy.
  • Knowledge of multi-agency services available and processes for referral.
  • Knowledge of the 2 effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
  • An awareness of risk management.

Desirable

  • Knowledge of local health needs strategies.
