Health Care Assistant Job summary
The CAMHS service for Gwynedd and Ynys Mon areas are seeking a band 3 Health Care Assistant to work within a new Transition team covering both counties. This is a new role and will be community based. You will be supporting young people, mainly requiring Secondary care provision, between the ages of 17-19 who are transitioning from CAMHS to Adult Mental Health Services. The team will consist of a CAMHS Practitioner, a Transition Lead, a Health Care Support Worker and a Team Lead who will delegate tasks on a day to day basis. It is a role likely to evolve over time.
Main duties of the job
The health care assistant main tasks will include practical support for those young people, mentoring and coaching to help them to develop new skills and access continued intervention. You will have excellent engagement skills, be enthusiastic, motivated, working collaboratively with young people to complete their transition passport/handover plan. You will work in accordance to best practice where transition between the services takes place. Your role involves both pre and post transition intervention for as long as it is required. A key part of your role will be liaison with the Patient Experience team where you will be proactive in seeking experience of transition feedback, collating this data and supporting implementation of an action plan to improve their transition experience.
The ability to speak Welsh is essential for this post.
