We are looking for individual Health Care Assistant who would like to be part of a dynamic and forward thinking team. Do you enjoy the challenge of a wide variety of work? Could this be you?
The role of a community Health Care Assistant is to visit patients at home to provide nursing interventions under the direction of registered nursing colleagues. The focus of the role is to support admission avoidance, to promote independence and to assist in the effective management of patients with long term conditions.
The Health Care Assistant posts would suit someone interested in integrated working and wanting to achieve the best for their patients. They will work within a multi -professional team to provide individualised patient care.
We are looking for a motivated Health Care Assistant, who are dynamic and keen to work as part of a team. The successful candidates will need to have some relevant experience, good communication skills and the ability to work well as a team member.

Applicants for Health Care Assistant must have access to a vehicle for work and possession of a full UK driving license is essential.

Main duties of the job
To provide a supporting role to registered practitioners in the Integrated Community Team in the delivery of nursing care respecting patients’ privacy and dignity at all times.
To act as key worker for patients under the direction of the registered practitioners.
To assist in planning, delivering and evaluating programmes of care prescribed by the registered practitioners, regularly liaising at the beginning and throughout each shift.
To observe, monitor and record patient activity and health status as appropriate, communicating any changes in their condition to the registered practitioners.
To be responsible for progressing patient’s treatment plans within guidelines set by registered practitioners and regularly contributing to the review of each patient’s progress within the team
To perform a range of clinical nursing skills as directed by the registered practitioners:
Skin care/Pressure Ulcer prevention
Vital Signs – Temp/Pulse/Lying and Standing BP/Respiratory Rate
Mouth care/Eye care/Toe nails and Finger nails
Catheter care/Bladder washouts/Stoma care
Catheterisation (under registered practitioner discretion — NVQ III Training)
Continence re-assessment
Bowel care (suppositories)
Venepuncture
Basic Wound care
Simple bandaging and paste bandaging
To provide information and advice to patients and carers on how to optimise functional ability, nutrition, health and safety, and health promotion
