Job Summary Health Care Assistant
We are looking for a Community health care assistant worker likely with psychiatric experience, to follow up Older adult service users and carers after they’ve been assessed by a Senior member in the team presenting with comorbidity of mental health and physical health issue inclusive of social issues. The post holder will receive direct supervision from the Band 7 CNS where they can develop further in their role.
The health care assistant will aim to provide a flexible, responsive and pro-active programme of care and support for the service users and their carers. You will be required to link across a wide variety of statutory and non- statutory organisations such as Housing, community organisations as well as other voluntary and non -voluntary services. You will also be involved in specialist mental health assessments and the application of evidence based psychosocial interventions at a low level to provide safety for patients.
As a Band 4 Nurse within our team there are many opportunities for personal development, and learning opportunities around social care, physical health care, and MDT working, with an opportunity to take part in teaching and education from patient to carers and as well as third sector.
Main duties of the health care assistant
- Follow up older adult patients in the community who has been assessed by the team and needing support for their social care, physical care or mental health care needs.
- See to patient physical health needs that is impacting on patient’s mental health i.e blood pressure, weight loss, stress
- Support carers and advising them about available support in the community
- carry out simple care needs assessment and record keeping on epjs
- work autonomously with support from their supervisor/ manager of the team
- Risk assess patient in the community for the older adult population
- Support community patient to access their GP in order to reduce inequality in services
- Support carer and signpost them to community support available
