Support Worker Job Summary
An exciting support worker opportunity has arisen for an experienced health care worker who holds a Level 4 Vocational qualification to support the Macmillan Rarer Tumour Nurse Specialist, providing high quality patient centred care to those referred with a suspicion of a rarer cancer
The support worker will act as an assistant to the Macmillan Rarer Tumour Nurse Specialist, undertaking an assessment of patient/carers holistic needs, providing general information help and support from the point of diagnosis and throughout the cancer pathway.
The post holder will be the first point of contact for patients/carers, identifying and addressing their general holistic needs, signposting to other services/agencies, alerting the Macmillan Clinical Nurse Specialist when patients require specialist input.
The post holder will also support the patient pathway by chasing and bookings appointments and provide holistic support for patients who attend regularly for lifelong treatment who do not require specialist support
The role will be required to undertake be some administrative aspects to support the service including data entry onto the Somerset Cancer Registry
Main duties of the job
We welcome applications from caring, motivated Health/Social Care Workers who have experience working with cancer patients. Educated to GCSE level in English and Mathematics. Must possess a vocational qualification at level 4 to be considered for this post and hold or be willing to undertake the Foundations of Cancer Care or equivalent in line with the National Cancer Education Framework
Interested applicants must also be able to demonstrate good communication, interpersonal, organisational and IT skills.
To work under the direction of the registered nurse providing high quality care to patients and carers living with a rarer cancer
The role involves being responsible for aspects of direct care delivery, assessment of patient and carer needs and the provision of general information, help and support.
To be an extended member of site-specific MDTs, ensuring effective communication across the Trust, Primary Care and with statuary and voluntary agencies
To undertake the administrative aspects of the service, coordinating the patient pathway between organisations
