Salford Primary Care Together (SPCT)Community Interest Company(CIC) is looking to recruit a Practice Nurse to join our dedicated practice team within Salford.
The Practice Nurse role will be based across sites as per the needs of the business.
Previous experience of working in practice is preferable but not essential.
Part Time or Full Time hours available for the right applicant with the opportunity of flexible working hours that suit the needs of the business. Our practice opening hours are 8am – 8pm everyday. Occasional weekend work may be required.
Full clean drivers license and car essential for Home Visits.
Practice Nurse Main duties
We are seeking an experienced Practice Nurse who will fulfill core responsibilities of the role which will include:
Long Term Condition management
Cytology
Child Immunisations
Vaccinations
Contraception
Swabs, specimens, bloods, blood pressures
Keeping accurate and detailed health records on the Emis system
A key role in QOF/QIF achievement
This post holder will also uphold the Surgeries protocols and policies, carrying out their role with compassion, understanding and professionalism.
This role may support the wider team with:
Clinical audits
Safeguarding concerns
Support and participate in shared learning over the Practice
Support for GPs in patient care
Ability to review clinical practices and respond to national policies and initiatives where necessary
Participate in local initiatives in order to enhance local patient care where necessary

RESPONSIBLE TO: Lead Practice Nurse
ACCCOUNTABLE TO: Clinical lead
JOB SUMMARY:
To contribute to the successful delivery of the key outcomes of the SPCT business plan by provision of a high quality nursing service to a defined practice population. This will include assessment, health promotion, chronic disease management, health screening and treatment in line with the professional competence of the post holder. To be involved with the practice delivery of National Service Framework targets and local Health Improvement priorities. Identification of nursing needs of the population. Responsibilities to maintaining and improving QOF data in line with the business plan and Salford Primary Care Togethers objectives.
