Support Worker Job Summary
This support worker will be based across Intermediate Care Lambeth (Rehab & Reablement).
The purpose of this support worker service is to provide home based support and rehabilitation for clients, who are 18 years and over, to maximise independence. Clients will have a wide range of functional difficulties, long – term conditions and disabilities: they may have multiple pathologies.
We aim to prevent unnecessary hospital admission, facilitate hospital discharge and reduce reliance on long-term care where possible.
Short-term outcome focused interventions are delivered in client’s homes for a period of up to six weeks. This is achieved through multi-disciplinary (MDT) assessment and rehabilitation coordinated by Senior Therapists (Specialist Practitioners) and Nurses, supported by Rehabilitation Support Workers (RSWs).
The MDT comprises of: Physiotherapists, Occupational Therapists, Nurses, Pharmacy, Social Workers, Assessors, Rehabilitation Support Workers and a Specialist Geriatric Consultant.
The Service is an integrated partnership across health and social care and carries out the following function.
- Rehab and Reablement (R & R): To facilitate timely discharge home from hospital in order to maximise the potential for recovery and minimise readmission to hospital. Uses an enabling approach and goal orientated rehabilitation in order to maximize a client’s independent living skills and reduce the need for long-term care.
Support Worker Main duties
We are looking for Band 4 Senior Rehabilitation Support Workers to join us in our expanding and award winning services, which provide healthcare at home and prevent admission into our hospitals.
You will have access to on-going support within our services.

The people that we support have a range of diagnoses and clinical presentations, including falls, orthopaedic, respiratory, frailty and neurological conditions.

Job responsibilities
- To contribute to the assessment, planning, implementation and review of individual rehabilitation and care programmes for clients. These programmes may contain home exercises and/or relate to Activities of Daily Living with the aim of increasing client function and independence.
- To independently implement these rehabilitation and care programmes as appropriate in the patients property.
- Tasks may include (but are not limited to) teaching prescribed exercise programs, practicing transfers and mobility, assisting and promoting independence in personal care and domestic tasks, undertaking access visits to feedback environmental information to therapists and documenting interventions accurately in notes following Trust guidelines.
- To treat clients, often unsupervised, according to agreed care plans and protocols, independently making appropriate adjustments to the rehabilitation programme as required.
- To be responsible for monitoring clients progress and amend clients programmes independently in order to maximise rehabilitation progression and patient safety. This should be completed within own skill-set and professional limitations.
- To report back to the relevant professional or duty therapist or @risk nurses as required, regarding any changes in the clients condition following the local escalation procedure.
- To complete training in order to be able to assess for and provide basic equipment in order to support patient safety. This may be Trusted Assessor training or equivalent.
- To seek timely advice and support as appropriate from the senior practitioners or supervisors.
- To work as part of a flexible multidisciplinary team and undertake tasks delegated by the Occupational Therapists, Physiotherapists. Nurses and Social Workers and/or team leaders in a variety of settings primarily in the clients home or community.
- To communicate effectively with clients and their carers; particularly clients with impairments, which may affect their ability to communicate their needs and/or give informed consent to treatment.
- To support and develop RSWs within a competency framework.
- To supervise RSWs with support from senior staff. This may be on a one-to-one or group basis.
- To participate in the prioritisation and triage of referrals, based on given criteria and using departmental protocols. This is done independently, with guidance from clinicians as necessary. This may be referrals to any of the services and will require familiarity of the different pathways involved.
- To participate in clinical governance and service development activities and take a lead role for a delegated service development area.
- To participate in the planning and delivery of service development and/or teaching initiatives within the team.

Essential
- Understanding of a wide range of physical and psychological conditions and their impact on a client’s ability to undertake every day activities, mobility and communication.
- A basic understanding of Occupational Therapy, Speech and Language Therapy, Physiotherapy and Social Work, and the role of a support worker /Therapy Assistant in supporting the rehabilitation and assessment team.
- Competent in the use of basic IT including MS Word, email, internet and other administrative tasks
- Fit and able to perform duties requiring a moderate to intense level of physical activity over a sustained period of time including travel around the borough and clinical tasks.
- Able to demonstrate and use planning, organisational and time management skills to prioritise and organise client caseloads and competing time and workload demands.
- Ability to communicate and negotiate effectively with clients, carers and colleagues in order to achieve the best outcomes for patients.
- Ability to work autonomously and progress/make adjustments to client care/rehabilitation programmes.
Desirable
- Knowledge of equipment used for manual handling, encouraging independence at home and of minor adaptations.

- Relevant significant experience working in a health, education or social care setting
- Relevant significant experience working with adults and older people with a physical disability for at least one year

Qualifications/Knowledge

- Evidence of literacy / numeracy to GCSE level
- 3 A-levels or NVQ level 3 or equivalent skills in relevant subjects e.g biology, social care or Foundation Degree in Health and Social Care T-level or Similar relevant experience at level 3 apprenticeship, such as Senior Healthcare Support Worker

- Level 3 or 4 occupation specific qualification(s) – such as the Senior Healthcare Support Worker apprenticeship
- Competency based development and other role specific local training such as Trusted Assessor
